The move was confirmed by the Pretoria franchise on Tuesday afternoon.
Ismaiel, who left the Bulls at the end of the 2018 season to join English club Harlequins, will return home and join Jake White's side on a two-year deal.
The 26-year-old represented the Bulls between 2012 and 2018 and played one Test against Wales in Washington DC in 2018.
White has signed several high-profile former Springboks since becoming Bulls director of rugby, including wing Gio Aplon, prop Marcel van der Merwe and flank Arno Botha.
In addition, the Bulls also recently signed Cheetahs lock Walt Steenkamp and Western Province hooker Schalk Erasmus and unveiled former Stormers forwards coach Russell Winter as their new assistant coach.
