Milan - Italy flanker Mauro Bergamasco will miss the two-match Test tour to South Africa.
The Italian Rugby Federation confirmed on Saturday that the Stade Francais player had failed to recover sufficiently from surgery to repair tendons in his right hand.
Simone Favoro, who has won five caps, was called up as cover for the 31-year-old Bergamasco.
The team is set to be captained by No 8 Sergio Parisse, who has been out of action since tearing knee ligaments in November.
Parisse will take part in a practice match against a local side on Saturday to confirm his fitness.
